• What made you decide to work with me?
• What did you enjoy about the experience?
• How did working with me improve the end project?
• Would you recommend me to others?

These are just some suggestions.  It would be super helpful if you could create that review on Google.  But you could also email me with your response.  Whatever works best for you.
